For your convenience, you may either phone FAX, or submit your prescriptions to us using this form. Please note all information with an * is required to process the prescription.
Delivery Service
We ship via Golden State Overnight. All packages will arrive by 10:30 am (*) the morning after they are shipped, including Saturday. (**)
Email notification of package shipment is available .
(*) Depends on address in some rural areas.
(**) Delivery will still be overnight, but may not arrive until 2-3pm
